PHNCF Academic Award

To be eligible for the PHNCF Academic Award you must:

Be a sophomore, junior or senior year student enrolled at one of the participating Educational Institutions.

Be a student pursuing a degree/certification in the healthcare field.

Secure one letter of reference from faculty, employer, clergy or community leader regarding academic performance and contributions to the school and/or community (Letter must be secured by the student and attached to the application).

Submit a written essay as outlined within the Application.

Attach a copy of most recent GPA.

Abide by the guidelines and stipulations within the Application.

PHN Health Professional Academic Award

PHN/PHNCF employee an/or a spouse, dependent or step child of a Primary Health Network/Primary Health Network Charitable Foundation employee will not be permitted to apply for this academic award.

To be eligible for the PHN Senior Management Award you must:

Be a resident in one of following counties; Ashtabula, Trumbull, Crawford, Mercer, Lawrence, Beaver, Butler, Clarion, Armstrong, Westmoreland, Indiana, Cambria, or Blair.

Be a student pursuing a degree/certification in the healthcare field.

Secure one letter of reference from faculty, employer, clergy or community leader regarding academic performance and contributions to the school and/or community (Letter must be secured by the student and attached to the application).

Submit a written essay as outlined within the Application.

Attach a copy of most recent GPA.

Abide by the guidelines and stipulations within the Application.
