PHNCF van transportation means healthier children and peace of mind for families

The mission of the PHN Charitable Foundation is to build and support programs that increase access to healthcare services and to assist, through scholarships, the pursuit of medical and allied health careers.

The PHN Charitable Foundation was created in 2005 to complement and expand the mission of the Primary Health Network, a Federally Qualified Health Center (FQHC) (link) founded in 1984.

All PHN sites are located within Federally designated Medical Health Professional Shortage Areas, Medically Underserved Areas, and/or Dental Health Professional Shortage Areas.

Our joint vision is to provide the highest quality medical and dental care to patients; to provide rides to medical appointments for those without reliable transportation; and to reverse the shortage of medical professionals throughout the communities within the PHN service area.
